Sciatica During Pregnancy: Why It Happens and How Chiropractic Care Relieves It

Sciatica during pregnancy is incredibly common, incredibly uncomfortable, and incredibly undertreated. Most moms are told to rest, use a heating pad, and wait it out until after delivery.

What Is Sciatica During Pregnancy?
The sciatic nerve is the longest nerve in the body — it runs from your lower back, through your hips and buttocks, and down each leg. Sciatica refers to pain, numbness, tingling, or weakness that follows the path of that nerve.
During pregnancy, sciatica typically happens for one of two reasons:
Direct pressure. As your uterus grows and your baby shifts position, they can press directly on the sciatic nerve — especially in the third trimester. This tends to cause sudden, sharp, shooting pain.
Pelvic and spinal misalignment. This is the more common cause, and the one chiropractic care addresses most effectively. As your center of gravity shifts and your posture changes throughout pregnancy, the pelvis and lumbar spine can become misaligned. This creates tension and compression around the sciatic nerve even when baby isn't directly on it.
The hormone relaxin — which loosens your ligaments to prepare for birth — also contributes, because it allows the pelvis to shift in ways it normally wouldn't, increasing the chance of imbalance and nerve irritation.
Sciatica vs. Round Ligament Pain — How to Tell the Difference
These two get confused constantly, so here's a quick way to tell them apart:
Round ligament pain is usually a sharp, quick, stabbing pain on one or both sides of your lower belly or groin — often triggered by sudden movement like standing up, rolling over, or sneezing. It doesn't travel down your leg.
Sciatica is felt in your lower back, buttock, and travels down the back or side of your leg — sometimes all the way to your foot. It can be sharp, achy, burning, or come with numbness and tingling. It's often worse with sitting and may persist rather than coming in quick jabs.
If your pain runs down your leg, it's sciatica. If it's sharp and localized to your belly or groin, it's likely round ligament pain. Both respond well to chiropractic care, but knowing which one you're dealing with helps us treat it more precisely.
Sciatica Trimester by Trimester
First trimester: Sciatica is less common this early, since baby isn't yet large enough to create direct pressure. If it shows up now, it's usually related to pre-existing pelvic misalignment that pregnancy hormones are starting to expose.
Second trimester: This is when many moms first notice symptoms. As your belly grows and your posture shifts to compensate, the lumbar curve increases and pelvic alignment starts to shift — increasing pressure on the sciatic nerve.
Third trimester: Symptoms are often most intense here. Baby is larger and positioning matters more — if baby is sitting in a posterior position or low against the sciatic nerve, direct pressure adds to the misalignment-driven irritation already present. This is also when many moms describe the pain as constant rather than occasional.
Understanding where you are in this progression helps set expectations — and reinforces why starting care earlier in pregnancy, rather than waiting until third-trimester pain is severe, leads to better outcomes.

Avoid sitting for long stretches. If you have a desk job, stand and move every 30 minutes. Prolonged sitting compresses the sciatic nerve directly.
Try a gentle figure-four stretch. Lying on your back (or propped up if that's more comfortable later in pregnancy), cross one ankle over the opposite knee and gently pull the supporting leg toward your chest. This can relieve piriformis tension that often contributes to sciatic pain.
Use a tennis ball for gentle glute release. Sitting on a tennis ball and gently rolling over the glute area (avoiding direct nerve pressure) can ease muscle tension that's compressing the nerve.
These can help in the moment, but they don't fix the underlying pelvic imbalance — that's where chiropractic care comes in.
